Health Risk Stratification Model: How Well Do You Manage ‘Falling Risk’ Populations?

Health risk stratification is scalable, according to the experience of Ochsner Health System, whose scaling and centralization of risk stratification and care coordination protocols across its nine-hospital system drive ROI and improve clinical outcomes and efficiency.

Here, Mark Green, system AVP of transition management at Ochsner Health System, explains why health plans and providers need better control over ‘falling risk’ patients.

Regardless of your patient population, no matter how small or large, you’ve got a segment of your population that are healthy patients. You’ve got a certain percentage, about 40 percent, who are at very low risk.

About 20 percent of your population falls into a ‘rising risk’ segment. Those are patients with chronic diseases who are somewhat adherent and compliant. You’ve got some that are newly diagnosed with depression, and a comorbidity. Then you’ve got this very top 3 to 5 percent, which are your poorly controlled multiple comorbidities that need your absolute highest touch, whether it’s through complex case managers or other programs that are the highest touch of those patients.

That is the typical model in the United States where you see this segmentation. In this country, we do a relatively good job of understanding ‘rising risk’ patients. Those are your patients that are diabetic, and suddenly you see their A1C go out of control. You know they’re going off-track for some reason, whether it’s compliance, adherence, needing medication adjustments, or some other social interactions happening outside your care model. These are your ‘rising risk’ patients.

As the country begins to understand this risk stratification, it understands the ‘falling risk’ patients, too. For example, we had a congestive heart failure (CHF) clinic that was pretty successful in managing patients; they had approximately 100 patients in their CHF clinic. They were taking these complex CHF patients and sending them through education and hooking them up with complex case managers. Pretty soon they filled their entire clinic up and didn’t have any more access for new patients. It failed pretty quickly because they weren’t able to churn these patients.

As we began to do a root cause analysis of why this happened, to understand why we didn’t see the sustainability in this program, we realized it was because we never moved patients down that risk stratification model. We kept them in there forever. We received them, we managed them and we got them better. But we never moved them down, so we never had room for another newly diagnosed, out-of-control CHF patient.

That’s a really critical step to understand: managing not only your rising risk but also your falling risk patient population within the sub-categories of your overall risk segmentation. It’s a living organism moving in and out of these different components.

Longitudinal Care Plans, Risk Scores Raise Patient Engagement for MSSP ACO’s Complex Population

A top-performing MSSP in 2014, the Memorial Hermann ACO has successfully engaged its Complex Care population via a collaborative care coordination approach.

The Memorial Hermann ACO may have been one of 2014’s top-performing Medicare Shared Savings Programs (MSSPs), but the health system’s commitment to achieving quality outcomes was solidified more than eight years ago, when its own physicians asked for a clinically integrated physician network.

Memorial Hermann complied, developing a set of tools, training and care models to not only support the physicians but also reflect payors’ needs: chief among them, initiatives that could boost patient engagement.

Today, the Memorial Hermann ACO has a patient-centered care delivery strategy built on teamwork and collaboration. The Texas ACO is proud to point to a patient engagement rate of 74 percent for individuals enrolled in Complex Care, an initiative for individuals with long-term, multiple chronic conditions that has significantly reduced cost and hospital lengths of stay for participants.

This patient engagement measure represents members who consent to participate in the program and remain engaged for 30 days, explained Mary Folladori, RN, MSN, FACM, CMAC, system director of care management at Memorial Hermann Physician Network and ACO, during Care Coordination in an ACO: Managing the Population Health Continuum from Wellness to End-of-Life, a September 2015 webinar from the Healthcare Intelligence Network now available for replay.

Ms. Folladori provided an overview of the ACO’s care coordination strategy that in 2014 generated savings of nearly $53 million in the MSSP program, resulting in a health system payout of almost $23 million. The ACO’s performance earned Memorial Hermann a MSSP quality score of 88 percent.

Some high points from Memorial Hermann’s ACO strategy include the following:

  • Embedding of care coordinators into the ‘micro culture’ of a physician practice, its community and the members served by the practice;
  • Strategic use of a data warehouse to identify vulnerable members early and link them with needed health services;
  • Development of comprehensive risk scores derived from multiple sources for Complex Care patients; and
  • Creation of longitudinal care plans that follow Complex Care patients for up to 18 months and help to transition them back to a baseline level of functioning.

In wrapping up observations on Memorial Hermann’s quality-driven approach, Ms. Folladori quoted its CEO, Chris Lloyd: “The success that has been found within our ACO is deeply based on a collaborative approach to care. It has been cultivated over eight years with our commitment to clinical integration. We all strongly believe that without that strong clinically integrated physician network, without our physicians driving those quality outcomes, we would not have been as successful as we have.”

With so much emphasis on quality and outcomes, it’s no wonder participation today in the Memorial Hermann ACO is by invitation only—and only after a practice has passed an assessment.

New Population Health Management Strategy for ‘Emergent-Risk:’ Arrest Trajectory of Compounding Conditions, Cost

For many years, healthcare organizations have invested in two approaches to population health management: First, wellness management for healthy populations who want to prevent illness, and second, disease/case management for the very sick who must adhere to physicians’ prescribed medical care.

Now, organizations are filling the gap between the healthy and very sick by investing in the ’emergent-risk’ population—adults with one or more pre-chronic or early-stage chronic conditions. Population goal: arrest the trajectory of compounding chronic conditions that compound declines in quality of life and compound increases in cost of care.

Consumer Engagement with Digital Health Self-Management

For many years, the natural worsening of chronic illness has been the focus of academic institutions, a few digital health innovators, and pioneering health plans. As a result of these efforts, a new class of digital health programs is now proven to arrest the trajectory of chronic illness. It started when Stanford’s Patient Education Research Center discovered that self-efficacy (a person’s belief in his or her own ability to achieve goals) had the strongest correlation to improved health outcomes. With this discovery, the concept of health self-management was born. Thus began the drive to engage and impact consumers.

Over the years, healthcare organizations across the United States began offering Stanford’s in-person workshops with notable improvements in health and with measurable reductions in cost of care. According to Stanford research, participants reduced pain, fatigue, depression, and A1C—and reduced ER visits and days in the hospital for up to one year.1 While results were impressive, the programs had one drawback for managers of population health: due to the cost and complexity of in-person delivery, the programs were hard to scale.

As web-based technologies advanced, the research community began to search for ways to harness digital innovation to scale evidence-based programs. In 2006, researchers at the University of Pittsburgh tested the first digital self-management program aimed at diabetes prevention. This digital translation2 of the NIH’s Diabetes Prevention Program3 (DPP) delivered outcomes that mirrored those achieved with the DPP’s in-person, self-management intervention—but at a fraction of the cost.

Data-Driven Insight: The Compounding Effect

As health plans began to adopt digital health self-management, data revealed deeper insights into individuals with prechronic and early-stage chronic conditions. Data from years of Canary Health research with a pioneering health plan shows that chronically ill patients add, on average, a new chronic condition every two to three years. These compounding conditions drive compounding increases in the cost of care—specifically in the areas of pharmacy, medical equipment, and outpatient care.

And without intervention, according to Advisory Board, each year 15 to 20 percent transition to the high-risk population of very sick individuals who require high-cost medical care. This trajectory of chronic illness translates into an additional $1,000-$3,500 in expenses per person, per year. With 80 million adults in this population, that’s an additional $80-$280 billion in costs each year to the U.S. healthcare system. If healthcare leaders don’t prevent this compounding effect, both health plans and providers will hit a financial tipping point where the cost of care puts both margins and mission at risk.

Proven Outcomes: Arresting the Trajectory of Chronic Illness

As health plans began to measure the ROI of digital interventions, a deeper look at results revealed the broader and longer-term impact4 of digital health self-management programs. For emergent-risk populations, the interventions accomplished the following:

  • Halted the progression of individuals’ preconditions to diabetes, heart disease and other conditions;
  • Slowed the progression of existing conditions, and;
  • Prevented compounding conditions and compounding costs of care.

On the heels of this research, the goal became “trajectory impact” at a population level: programs for the emergent-risk population are now designed to arrest the trajectory of compounding conditions and compounding costs of care. With digital technology’s ability to scale, entire emergent-risk populations can be targeted immediately for outreach and intervention.

And with the lower cost structure of digital technology, health self-management interventions can generate a return beginning one year after the intervention and continuing over the lifetime of each individual.

ROI and 12 More Rewards from Stratifying High-Risk, High-Cost Patients

Health risk stratification—for example, grouping diabetics in a single physician practice or drilling down to an ACO’s subset of medication non-adherent diabetics with elevated HbA1cs—followed by risk-appropriate interventions can significantly enhance a healthcare organization’s clinical and financial outlook.

For 9.4 percent of respondents to HIN’s 2014 Health Risk Stratification Survey, risk stratification resulted in program ROI of between 3:1 and 4:1, while 6.3 said return on investment was greater than 5:1.

Stratification and targeted interventions also generated a healthy drop in healthcare cost, nursing home stays, ER utilization and time off work while boosting quality ratings, patient engagement levels and care plan adherence.

Survey respondents further quantified successes achieved from health risk stratification in their own words:

AltaMed Constructs Business Case for Care Coordination Team

The AltaMed multidisciplinary care team targets dual eligibles with multiple chronic conditions and functional and cognitive impairments.

When the largest FQHC in the country set out to quantify the contributions of its multidisciplinary care team, it found the concept didn’t fit neatly into return on investment models.

So at budget time this year, leaders of AltaMed Health Services Corporation’s care coordination model for its highest risk patients identified seven performance metrics to present to its CFO, explained Shameka Coles, AltaMed’s associate vice president of medical management, during A Comprehensive Care Management Model: Care Coordination for Complex Patients, a May 2015 webinar now available for replay.

The evidence that ultimately secured funding for the care coordination project’s next phase included the model’s impact on specialty costs, emergency room visits, and HEDIS® measures, among other factors.

These were all areas examined early on, back in phase one, when the care coordination team set a number of strategic goals that aligned with the corporation’s five pillars: service, quality, people, community and finance.

Rolled out in four phases beginning in July 2014, the model is aimed at AltaMed’s dually eligible population— Medicare-Medicaid beneficiaries with high utilization, multiple chronic conditions, and multiple functional and cognitive impairments, Ms. Coles explained.

Phase one of the project was devoted to understanding and engaging the duals population via telephonic and print outreach, then developing a care management model reflecting both Triple Aim and patient-centered medical home goals. (The 23-site multi-specialty physician organization in Southern California has earned Joint Commission primary care medical home designation.)

At the heart of the model is a multidisciplinary care team, which counts a care coordinator, clinic patient navigator and care transitions coach among its eleven roles. Patients are stratified as high, moderate or low risk and matched to risk-appropriate interventions.

“Each member is activated based on where the patient is at in the continuum of care,” noted Ms. Coles, who also reviewed team member roles and responsibilities and a host of complementary programs supporting care coordination during the May 2015 program sponsored by the Healthcare Intelligence Network.

In phase two, focused on development of end-to-end workflows, staff assessments and ratios, and team training, AltaMed hired an educator, fleshed out the patient navigator role, and examined integration of behavioral health and long-term services and supports (LTSS).

Phase three triggered a deeper dive into case manager caseloads and utilization patterns as well as several quality improvement activities.

Now in phase four, the goal of AltaMed’s care coordination model is to ensure it can reflect a financial impact. “We’ll look very closely at our per member per month cost and our inpatient metrics,” Ms. Coles concluded.

  • Readmissions: More than half of survey respondents participate in post-acute partnerships, with home health collaborations the most common (79 percent). These partnerships serve to streamline processes and care transitions, educate and align staff, and implement changes of value to patients, said respondents to the fourth annual Reducing Hospital Readmissions Survey.
  • Palliative Care: While the majority of respondents (68 percent) administer palliative care on an inpatient basis, more than half (54 percent) say care is conducted on home visits and just under a third offer palliative care at extended care facilities.
  • Patient-Centered Medical Home: Prepared to take their medical homes to the next level of care in the year to come, almost half—48 percent—have joined or expect to join a medical home neighborhood, defined by TransformMed℠ as “a strong foundation of transformed primary care practices aligned with health systems and specialists to insure that care is maximally coordinated and managed.
  • Remote Patient Monitoring: More than half of 2014 respondents—54 percent—have instituted remote monitoring programs, the survey found, which was most often employed for patients or health plan members with multiple chronic conditions (83 percent). Other targets of a remote monitoring strategy included frequent utilizers of hospitals and ERs (62 percent) and the recently discharged (52 percent).
  • Telephonic Case Management: More than 84 percent of respondents utilize telephonic case managers. „One-fifth of telephonic case managers work within the office of a primary care practice.
  • Population Health Management: The last two years reflects a dramatic surge in the use of data analytics tools barely on population health management’s radar in 2012: the use of health risk assessments (HRAs), registries and biometric screenings more than tripled in the last 24 months, while electronic health record (EHR) applications for population health increased five-fold for the same period.
  • Emergency Room Utilization: Among populations generating the majority of avoidable ED visits, dual eligibles jumped nearly 10 percent in the last four years, from 2 to 11 percent, while other populations—high utilizers, Medicare and Medicaid—remained roughly the same. „„Chronic disease replaced pain management as the most frequently presented problem in the ER, at 54 percent.
  • Stratification of High-Risk, High-Cost Patients: The „LACE readmission risk tool (Length of stay, Acute admission, Charleston Comorbidity score, ED visits) is considered the primary indice and screen to assess health risk, according to 33 percent of respondents.
  • Embedded Case Management: Fifty-seven percent of respondents embed or co-locate case managers in primary care practices, where their chief duties are care and transition management, reducing hospital readmissions and patient education and coaching.
  • 2015 Healthcare Forecast: Almost 92 percent of 2015 respondents said the impact of value-based healthcare on their business has been positive, with more than one quarter identifying healthcare’s value-based shift as the trend most likely to impact them in the year to come.

Top Risk Stratification Tools for Telephonic Case Management

The case management assessment is the top tool for stratifying candidates for telephonic case management contact, according to market data from HIN’s 2014 survey on Telephonic Case Management.

Sixty-one percent of respondents use a case management assessment to identify high-risk, high-cost individuals who would benefit from telephonic follow-up and care coordination.

Other risk stratification tools reported by survey respondents include the following:

  • Provider referral: 60 percent

  • Claims utilization data: 55 percent

  • Hospital census and discharge reports: 48 percent

  • Predictive modeling: 39 percent

  • Self- or family referrals: 37 percent

In other market data, more than 84 percent of respondents utilize telephonic case managers, with more than half—54 percent—making contact with patients from virtual home offices.

The complex comorbid are the primary targets of telephonic case managers (TCMs), the survey found, but the newly discharged, those in acute stages of chronic illness, frequent utilizers and high-risk, high-cost patients also receive their fair share of telephonic attention from these case managers.

Most critical to the stratification process is clinical patient data, say an overwhelming 87 percent of respondents to the Healthcare Intelligence Network’s (HIN) inaugural survey on Stratifying High-Risk Patients. However, obtaining and verifying patient data remain major challenges for many respondents. Following are 10 more statistics from our survey.

  • „„Hospital readmissions is the metric most favorably impacted by risk stratification tools, according to a majority of respondents.
  • „„In addition to high utilization, clinical diagnosis is considered a key factor in stratifying high-risk patients, according to 16 percent of respondents.
  • „„Case management as a post-stratification intervention is offered by 83 percent of respondents; health coaching by 56 percent.

Ochsner’s Standardized Risk Stratification, Care Coordination Protocols Boost Outcomes across Continuum

Ochsner Health System’s scaling of a successful transitional care model across one region not only reduced duplication of calls to recently discharged patients but also quadrupled its connect rate—from about 20 percent to nearly 98 percent of discharged patients—and decreased rehospitalizations by about 15 percent.

All while remaining salary-neutral.

To achieve these results and others like them, Ochsner uniformly applied scripts, templates and protocols to processes across its care continuum, even assuming clinical oversight for some providers in external facilities to ensure standardization, explained Mark Green, assistant VP of transition management at Ochsner, during Moving the Metrics: Financial and Quality Returns from System-wide Care Coordination and Risk Stratification, an October 2014 webinar now available for replay.

To replicate these achievements, the nine-hospital system looks up and down its continuum for opportunities to collaborate in care coordination and has elevated its approach to risk stratification. This culture shift is a prerequisite for success in today’s value-based climate, Green estimates.

“A really critical step to understand is managing not only your ‘rising risk’ but also your ‘falling risk’ patient population,” he said, categorizing ‘falling risk’ as those whose conditions are under control and who can be handed off to a lower risk medical home or chronic disease management environment.

Healthcare doesn’t currently do a good job of moving ‘falling risk’ patients down the stratification model, he said, which leaves little room for newly diagnosed ‘rising risk’—an out of control CHF patient, for example.

Risk stratification is scalable, Green emphasized, from single providers without an electronic medical record to a large health system or accountable care organization. As a nine-hospital system, Ochsner’s risk segmentation approach relies heavily on automation and data analytics. For example, every Ochsner hospital patient is assigned a severity of illness (SOI) level that helps to guide individuals to the appropriate level of care. For example, all level 3 patients are automatically referred to complex case management.

During the webinar, Green shared several of Ochsner’s collaborations in risk stratification and care coordination, including an automated post-discharge telephonic follow-up for emergency department patients that replaced its siloed approach and has reduced avoidable ER use in the range of 13 to 15 percent depending on the payor and the location.

10 Things to Know About Population Health Management in 2014

A population-based approach to health and care management is sustainable, say 96 percent of respondents to the latest Population Health Management (PHM) Survey by the Healthcare Intelligence Network, with home visits and social media expanding PHM’s reach along the care continuum.

In the two years since HIN last administered this survey, PHM teams have narrowed their approach, targeting individuals with moderate and complex health risks more closely than two years ago. The same period reflects a dramatic surge in the use of data analytics tools barely on PHM’s radar in 2012.

Here are more metrics derived from the 2014 Population Health Management survey:

  • The use of health risk assessments (HRAs), registries and biometric screenings more than tripled in the last 24 months, while electronic health record (EHR) applications increased five-fold for the same period.
  • PHM team composition has also shifted to reflect a greater reliance on primary care: primary care physicians (PCPs) appeared on 86 percent of respondents’ PHM teams, versus 60 percent in 2012.
  • „The prevalence of population health management programs remained relatively stable from 2012 to 2014, with just over half of respondents reporting PHM initiatives.
  • „„Almost two-thirds of 2014 respondents—64 percent—identified the HRA as the primary tool for determining appropriate PHM intervention levels, replacing claims data, the preferred stratification tool in 2012.
  • „„Almost one-third of responding PHM programs belong to a medical neighborhood, a new metric derived from this year’s survey.
  • „The use of remote patient monitoring in PHM nearly tripled from 2012 to 2014—from 13 percent to 38 percent.
  • One-quarter of PHM programs use social media to engage members, a new metric derived from the 2014 survey.
  • „„In a PHM metric new for 2014, 43 percent of PHM programs incorporate home visits into population health management plans.
  • Patient satisfaction is the key metric to evaluate PHM success, say 79 percent of respondents.
