CMS data shows that 30-day hospital readmission rates for Medicare beneficiaries began to decrease in 2012 — from 19.5 percent to 18.5 percent — but it is still a long way from the CMS goal of reducing preventable hospital readmissions by 20 percent, according to a new infographic from Vree Health.

This infographic compares 2011 to 2012 readmissions by state and conditions including CHF, AMI, pneumonia, and hip and knee replacements.
