A Coordinated Discharge Planning Approach to Reduce Avoidable Hospital Readmissions


Sharing the latest literature on the causes and prevention of hospital readmissions is Susan Shepard, the director of patient safety education for The Doctors Management Company. Shared Savings in the Medical Home
